TERMS
AND CONDITIONS OF TRANSFER OF MEMBERSHIP
The following terms and conditions must be adhered to
before the transfer would be effected:

LICENSE
TRANSFER APPLICATION FORM. The transferor
is required to fill in and sign the License Transfer
Application Form. |
| 2) |
TRANSFER
FEE. RM3,500
(Ringgit Malaysia Three Thousand and Five Hundred),
10% from the prevailing value at RM35,000 for Individual
Membership. RM9.000 (Ringgit Malaysia Nine Thousand),
10% from the prevailing value of RM90,000 for Corporate
Membership (three nominees) . |

OUTSTANDING
AMOUNT.
The transferor is required to settle all outstanding
amount of the membership fee and any other miscellaneous
charges due to the company prior to the transfer.

| 4) |
MEMBERSHIP
CARD AND CAR STICKER.
The transferor is required to surrender his or her
membership card (inclusive of spouse and children,
if any) and car stickers. |
| 5) |
LICENSE
AGREEMENT.
The transferor must surrender the Original License
Agreement. If in case of lost, transferor must made
declaration at the Commissioner of Oath or legal representative.

| 6) |
REDEMPTION
LETTER FROM FINANCIERS.
For member taking from the bank or finance Company,
a redemption letter from the financier is required
if it has not beef obtained earlier. |
| 7) |
TRANSFER
DURING SUBSISTENCE OF NOMINATION.
No transfer will be allowed during the subsistence
of any nomination. |

LICENSE
TRANSFER APPLICATION FORM.
The transferee is required to fill in and sign the
License Transfer Application Form. |

MEMBERSHIP
APPLICATION FORM.
To be completed and signed by the transferee. |

PROCESSING
FEE
at RM30.00 (Ringgit Malaysia Thirty only). |
| 4) |
REFUNDABLE
SECURITY DEPOSIT
at RM1,500 (Ringgit Malaysia One Thousand and Fine
Hundred). |
| 5) |
PHOTOGRAPHS.
The transferee is required to submit three (3) pieces
of passport-size photographs of the transferee, the
legal spouse and children below 18 years old, which
will be issued with cards. Please indicate name at
the back of each photograph. |
| 6) |
IDENTITY
CARD / PASSPORT / BIRTH CERTIFICATE.
The
transferee is required to submit photocopy of Identity
Card or Passport for himself, legal spouse and Birth
Certificate of his children below 12 years old. |
| 7) |
PAYMENT.
All payment by cheque must be made payable to
Berjaya Hills Resort BERHAD. |
